Lottery Project Funded by The National Lottery Community Fund. Improving Lives Notts has a project entitled Your Journey Forward which provides practical support to those living in the city diagnosed with a Severe Mental Illness and who are also experiencing two other intersecting forms of Severe Multiple Disadvantage such as Domestic Abuse/ Sexual Violence, harmful Substance Use, significant risk of Homelessness and/or contact with the Criminal Justice System. The project which has been made possible due to 3 years of funding from the National Lottery Community Fund is able to take 100 referrals each year. Your Journey Forward will provide tailored help for up to 12 months to those referred via the primary care pathway and beneficiaries will receive support to apply for benefits, access specialist debt advice and a whole a range of other practical assistance that will help overcome barriers preventing a person better managing their own mental and physical wellbeing.
