Who can refer?


Currently our service is funded to support people in Nottingham City who are receiving support in primary care services and who are NOT open to secondary care. Our referral criteria require individuals to have a Severe Mental Illness diagnosis and to be experiencing two other intersecting forms of Severe Multiple Disadvantage (SMD) such as Domestic Abuse/ Sexual Violence, harmful Substance Use, significant risk of Homelessness and/or contact with the Criminal Justice System. Support can be for up to 12 months, depending on the individual’s needs. This may include, but is not limited to:

  • Emotional wellbeing and mental health support, including building healthy routines and self-care practices.

  • Attending and managing appointments.

  • Budgeting and financial management.

  • Navigating benefits and completing applications, including PIP forms.

  • Accessing regular peer social groups..

  • Digital skills and online access.

  • Where appropriate, Signposting and/or supporting to access specialist services.

We will accept referrals from healthcare professionals providing support within primary care, such as from the GP or those in ARRS roles. Due to having a short waiting list, we cannot currently consider referrals outside of our full criteria.

We will email to confirm that we have received the referral and let you know if there will be a wait for the service.

When we are able to allocate a support worker they will contact you to make you aware and to get any more information, if this is needed.

> I want to refer someone who is in primary mental health services, but lives outside of Nottingham City. Can I make a referral?

Unfortunately, we are only funded to support people in Nottingham City at present.

> How frequently does your support take place?

All of our support is tailored to an individual’s needs and goals. However, given our capacity, we are not able to provide multiple visits a week and we are not a substitute for a social care package.

We have a weekly social cafe that includes one walking session per month, alongside a wide variety of social and creative groups such as pool, arts & crafts, jewellery making, music appreciation, crochet and coffee in the community.
